Valid Occupational Therapist license in Michigan

Training and Other Requirements

Orientation Safety Manual acknowledgement, updated annually
Mandatory Reporter training, updated annually
FERPA, updated annually
School-specific fingerprint instructions must be followed

Health Requirements

Physical exam, if required by the client
Negative TB test or chest X-ray, if required by the client or state
TB questionnaire, if required by the client, state, or due to past positive TB results
Drug testing, if required by the client
Flu vaccination/documentation, if required by the client

Notes

The school requires fingerprints, and the candidate cannot start while fingerprints are in process.
