Physical Therapist

Overview

Contract role in Sunnyvale, California
Home health setting
Day shift, 8:00 AM–5:30 PM
40 hours per week
Schedule may be flexible based on patient availability
One weekend day per month

Required Qualifications

Valid Physical Therapist license in California
Physical Therapist (1 year of experience)
Physical Therapist experience within the last year
BLS
Experience in a(n) Home Health setting

Role Highlights

Evaluate and treat patients using prescribed physical therapy programs
Restore function and help prevent or minimize disability after disease or injury
May supervise Physical Therapy Assistants
May complete initial admission/evaluation visits and report OASIS scores within required timelines
Document observations, clinical findings, interventions, goals, and discharge plans

Service Area

Sunnyvale and nearby cities, including Atherton, Cupertino, East Palo Alto, Los Altos, Los Altos Hills, Menlo Park, Mountain View, Palo Alto, Portola Valley, Santa Clara, Stanford, and Woodside
