Physical Therapist — Home Health
Overview
- Contract role in Albuquerque, New Mexico
- Home health setting
- Schedule: Monday–Friday
- Hours: 30 per week
- Contract length: 13 weeks
- Service area includes patients within a 45-mile radius of Albuquerque
Required Qualifications
- Bachelor's degree in physical therapy
- Valid Physical Therapist license in New Mexico
- BLS
- Driver's License
Experience
- Two years of clinical experience preferred
- Home health experience is a plus, but not required
Key Responsibilities
- Provide physical therapy services within the legal scope of practice
- Complete evaluations, progress notes, reassessments, recertifications, and OASIS documentation
- Develop, implement, and update individualized plans of care
- Deliver skilled interventions and maintenance therapy programs
- Supervise Physical Therapist Assistants and Home Health Aides as allowed by state practice acts
- Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies
- Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress
- Participate in quality improvement, interdisciplinary meetings, case reviews, and continuing education
