Physical Therapist - Home Health

Overview

Full-time contract role in home health
Day shift
36 hours per week
Service area includes White River Junction, Lebanon, Bradford, and Windsor

Required Qualifications

Bachelor’s degree in Physical Therapy
Valid Physical Therapist license in Vermont
1 year of experience as a Physical Therapist
Physical Therapist experience within the last year
BLS
Experience in a Home Health setting

Role Responsibilities

Evaluate patients and develop treatment plans
Carry out treatment programs according to agency policies and professional standards
Communicate with the health care team
Document evaluations, treatment goals, and objectives
Teach patients and family members about treatments and procedures

Work Setting

Home health
Average visit length is 45 minutes
Broad independence while working as part of a patient-care team
