Physical Therapist

Overview

• Contract role in Tuba City, Arizona
• Monday–Friday, 8am–5pm
• Orientation every 2 weeks

Required Qualifications

• Valid Physical Therapist license in Arizona
• Physical Therapist (3 years of experience)
• Physical Therapist experience within the last year
• BLS issued by American Heart Association
• Bachelor's degree in Physical Therapy
• Experience in a(n) Outpatient setting

Clinical Scope

• Provides physical therapy evaluation and treatment for inpatients and outpatients across the lifespan, from birth through geriatric
• Includes specialty services such as wound care, prosthetic and orthotic clinic, geriatric clinic, high school clinic, functional clinic, spinal cord clinic, assistive technology/adaptive equipment evaluation and treatment, and Durable Medical Equipment Prosthetic Orthotic Supply Program
• Includes management and implementation of the Physical Therapy program, quality improvement, risk management, safety, policies and procedures, Joint Commission preparation, staff education, reporting, staff meetings, and in-services
