Occupational Therapist for Home Health
Overview
- Contract role in Missoula, Montana
- 1.0 FTE / 40 hours per week
- Day shift
- Home health setting
Required Qualifications
- Valid Occupational Therapist license in Montana
- Occupational Therapist (2 years of experience)
- Occupational Therapist experience within the last year
- Bachelor's degree or higher in Occupational Therapy
- Experience in Supervisory roles
- Driver's License
Preferred Qualifications
- Two years of previous OT experience
- BLS/BCLS
Responsibilities
- Evaluate patients and establish plans of care in the home environment
- Provide direct patient care based on the care plan and physician orders
- Coordinate care with other disciplines and seek orders for additional services as needed
- Use occupational therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation in evaluations, re-evaluations, and treatment techniques
- Modify treatment as appropriate, including frequency and duration of care
- Score CMS-required OASIS and collaborate with the Quality team on needed adjustments
- Maintain understanding of insurance requirements in home health
- Participate in multidisciplinary conferences and communicate with physicians and other team members
- Document skilled services, adverse events, and relevant communications in the medical record
- Participate in Quality Assurance Performance Improvement initiatives
- Meet productivity standards
- Maintain accurate time logs and mileage
- Communicate with the scheduler each day worked
- Maximize continuity of care and efficient travel routes when preparing schedules
- Oversee services provided by the Occupational Therapy Assistant and perform supervisory visits within the required time frame
- Take supervision of CNA when other disciplines are no longer involved in the care plan
- Plan admissions, OASIS time points, and discharges throughout the patient’s episode of care
- Maintain occupational therapy licensure requirements, including continuing education standards
- Follow ethical standards defined by the occupational therapy licensure board and agency policy
- Participate in agency-provided educational opportunities
- Perform other duties as assigned
