Job Description

• CVHCare is seeking a qualified Occupational Therapist. New graduates are welcome to apply. Home health experience is preferred but not required.
• Assigned County Territory:

  • Area #11: Santa Clara, Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Saratoga, Los Gatos
  • Area #12: San Jose

Work Options

• Guaranteed Hours:

  • Minimum productivity of 27 points per week.
  • 36 hours guaranteed.
  • Must complete 30 points in the week.
  • Willingness to travel outside the area or work weekends is required to maintain weekly visit expectations.
• Non-Guaranteed Hours:

  • Weekend coverage may be required.
  • May need to go outside the service area to secure a full caseload.
  • No guaranteed hours; travelers can refuse patients.
  • Must be willing to see COVID+ and COVID PUI patients.

Visit Types and Points System

• OASIS ReCertification, Routine, Evaluation, Discharge, and other visit types convert to points and hours as follows:

  • 1 point = 1.33 hours.

Credentials Required

• Current California Occupational Therapist License
• BLS Certification
• Valid Driver's License
• Auto Insurance
• Auto Registration
• Annual TB Test
• MMR, Varicella, Hepatitis B, TDap (10 years), and Annual Flu Vaccination
• Drug Screen
• Physical Examination

Additional Information

• All requested time off (RTO) must be disclosed at the time of submission.
• Agencies must ensure that their contract mirrors the confirmation page to avoid removal from the client's vendor set.
