Clinical Care Registered Nurse (CCRN) - Battle Creek, Michigan

Job Summary

The Clinical Care Registered Nurse (CCRN) provides direct nursing care to participants, working independently with advanced nursing knowledge and skills to manage complex clinical situations. The role involves collaboration with both licensed and non-licensed personnel.

Key Responsibilities

Triaging emergent situations independently using advanced nursing knowledge
Monitoring acute and chronic medical conditions in coordination with Primary Care Providers (PCPs)
Utilizing care plans to provide clinic and home-based direct care
Administering treatments, procedures, and medications as prescribed
Conducting regular home visits to provide care and treatment as ordered by PCP, including wound care, lab draws, nurse assessments, and other nursing tasks

Qualifications

Associate degree in Nursing required; Bachelor’s degree preferred
Nursing Diploma or Degree from an accredited School of Nursing or University
Minimum of 1 year experience working with frail, elderly, or long-term care populations
Preferred: Minimum of 1 year home care experience
Preferred: Minimum of 1 year wound care experience

Specific Requirements

Current CPR certification by AHA or willingness to obtain it
Demonstrates core values: compassion, integrity, teamwork, and excellence

Work Environment

Visible presence in the center with frequent interruptions
Communication with staff, participants, caregivers, and other organizations as needed
May work beyond normal hours, including weekends and holidays

Minimum Working Requirements

Reliable transportation, valid Michigan driver’s license, and car insurance
Must be 18 years of age or older
Fully vaccinated or willing to be vaccinated against Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B
