Description
POSITION SUMMARY
Under general administrative direction, coordinates referrals from physicians, case management, and health care facilities for patients. Responds to and performs pre-admission screening for patients referred to the rehab program. Provides a significant level of patient education related to illness and rehabilitation recovery process. Responsible for guiding patients with rehabilitation DRGs through the discharge rehabilitation care continuum. Responsible for the development and implementation of referral program as a means to promote the feature and benefits of the rehab program to patients, families and the community. Collaborates among all disciplines to
ensure that patient’s receive care that consistently follows evidenced based standards. Completes established competencies for the position within designated introductory period. Other related duties as assigned
MINIMUM EDUCATION: Bachelor’s Degree from accredited college or university in health care clinical field.
PREFERRED EDUCATION: Master’s Degree from accredited college or university in nursing
MINIMUM EXPERIENCE: 2 years of clinical experience in health care environment, preferably in hospital .
Possess strong interpersonal skills and experience working with adult rehab patients and geriatric disabled
population.
PREFERRED EXPERIENCE: Marketing experience. Working knowledge of computers and basic software
applications used in job functions such as: PC, LAN, Allscripts, E-mail, Microsoft Office, ACCESS, Printers, Fax
and other machines.
REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State License in Clinical Specialty
PREFERRED CERTIFICATIONS/LICENSURE: N/A
REQUIRED COURSES/ COMPLETIONS (e.g., CPR): Must have active healthcare provider Basic Life
Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire
