Description
Clinical Pharmacist, Full Time – North Central Baptist: Hybrid (On-site and Remote Order Verification)
Candidates will work 7 on - 7 off, rotating through onsite overnight coverage and remote order verification alternating weeks.
The Clinical Pharmacist is responsible for maintaining pharmaceutical dispensing functions, including clinical review functions, in accordance with all legal and regulatory requirements. The Clinical Pharmacist provides pharmaceutical care and products to all neonate, child, adolescent, adult, and geriatric patients, including inpatients and outpatients.
General Functions:
- Interprets medication orders (verbal and written) and transcribes to computerized patient mediation profiles. Maintains patient medication profiles.
- Compounds and dispenses pharmaceuticals, including sterile, chemotherapy and parenteral nutrition products
- Dispenses controlled substances to patient care areas and maintains records
- Maintains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
- Provides direction to pharmacy personnel and ensures effective operation of the pharmacy department. Verifies the daily activities of pharmacy technicians, interns and residents.
- Complies with standards or rules and regulations of regulatory agencies: TJC, TX State Board of Pharmacy, TX Department of Health, OSHA, DEA, TX Department of Public Safety, EEOC / TEC.
- Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures and protocols
- Detects and reports suspected adverse drug reactions and medication occurrences.
- Utilizes formulary initiatives and clinical review to provide adequate pain management for the patient.
- Utilizes principles of drug dosing in the pediatric and neonatal patient to provide medications to patients in correct doses and formulations
- Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
- Adjusts dosages or choice of medications based on decreased renal clearances in the elderly or other patients with compromised renal function.
- Assists with pharmacokinetics consult service and drug dosing per BHS protocol
- Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.
- Reads extracts and interprets information in patient charts.
- Provides clinical consultations and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff.
- Provides drug information to the professional staff, patients and families.
- Participates in the development and presentation of orientation, education, and training programs to the pharmacy, medical, nursing, and other staffs.
- Follows up with other staff as a team to complete unresolved interventions.
- Performs initial screening of order entry to provide information to perform interventions and documents all clinical activities and interventions
- Utilizes resources efficiently and effectively
- Maintains safe environment
- Addresses patient population-specific needs
- Participates in Performance Improvement activities
MINIMUM EDUCATION: Graduate of accredited College of Pharmacy
MINIMUM EXPERIENCE: Hospital Internship Rotation
PREFERRED EXPERIENCE: Hospital experience
REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State Board of Pharmacy License
PREFERRED CERTIFICATIONS/LICENSURE: IV Admixture Certification
