Tripod Partners is seeking an experienced RN Care Manager for a Home Health role (non-hospital setting) covering Lake County. This position involves coordinating patient care, conducting in-home visits, and managing care plans across a diverse patient population.

Candidates must have at least 2 years of nursing experience, including recent Home Health experience, along with a background in care management. Proficiency in HCHB (Homecare Homebase) and OASIS documentation is required. An active RN license in the state of practice and AHA BLS (or equivalent) certification are also required.

A BSN and case management certification are preferred.
