Tripod Partners is seeking an experienced Home Dialysis Registered Nurse (RN) to support a well-established home therapies program serving patients across multiple outpatient clinic locations in New York.

This role focuses primarily on Peritoneal Dialysis (PD) patients, with additional support for Home Hemodialysis (HHD) patients. The Home Dialysis RN is responsible for patient assessments, ongoing clinical management, modality education, patient training, care coordination, and troubleshooting treatment-related concerns. The nurse will help patients achieve and maintain independence with their home dialysis therapies while providing ongoing clinical support and guidance.

Patient care is primarily clinic-based, with occasional travel to patient homes for initial assessments and select follow-up visits. Travel between clinic locations may also be required, and a personal vehicle is necessary.

The schedule consists of four 10-hour shifts, Monday through Friday, from 6:00 AM to 4:30 PM. Nurses participate in a rotating on-call schedule, typically every other week, including overnight weekday coverage and 24-hour weekend call responsibilities.

Candidates should be comfortable working independently after a brief orientation period and managing a small home dialysis patient population. Previous home dialysis experience is strongly preferred, with Peritoneal Dialysis experience highly desirable. Strong assessment, triage, patient education, and critical thinking skills are essential for success in this role.
