Position Overview
NorTek Medical Staffing is seeking experienced and compassionate Licensed Vocational Nurses (LVNs) or Registered Nurses (RNs) to join its expanding Houston-based team in a care management and clinical oversight capacity. This full-time, field-based role focuses on patient assessments, care coordination, and caregiver oversight within a non-medical in-home care setting.
This position requires daily travel to client homes throughout the Houston area and offers a consistent caseload with immediate start availability due to rapid growth.
Key Responsibilities
Travel to client homes across Houston to conduct initial and ongoing assessments
Manage a daily caseload of 5+ patients per day
Develop, review, and update individualized care plans
Provide clinical oversight and guidance to caregivers, CNAs, and PCAs
Serve as the primary clinical liaison for patients and families
Monitor care quality, documentation accuracy, and compliance standards
Identify changes in patient condition and escalate concerns appropriately
Support caregiver training, education, and performance oversight
Maintain accurate and timely documentation using EMR systems
Required Qualifications
Active LVN or RN license in the state of Texas
Experience in home-based care, hospice, geriatrics, or care management preferred
Strong assessment, communication, and organizational skills
Comfortable with daily travel to multiple client residences
Ability to manage a fast-paced, field-based schedule
EMR proficiency and strong documentation skills
Work Schedule
Full-time, field-based role
Daily travel required to patient homes
Average caseload: 5+ patients per day
Compensation & Benefits
LVN Pay: $55+/hour DOE
RN Pay: $65+/hour DOE (higher rates available based on experience)
Mileage reimbursement for all required travel
