Position Overview
NorTek Medical Staffing is recruiting an experienced Geriatric Nurse Practitioner (NP) to join a Program of All-Inclusive Care for the Elderly (PACE) in Michigan. This permanent role focuses on providing comprehensive, patient-centered care to older adults, helping them maintain independence while living safely in their communities.
Key Responsibilities
Provide primary and geriatric care to participants age 55+
Conduct comprehensive assessments, develop care plans, and manage chronic and acute conditions
Prescribe and manage medications and therapies
Collaborate with physicians, nurses, social workers, therapists, and other interdisciplinary team members
Ensure care aligns with CMS, Michigan Department of Health and Human Services, and PACE program standards
Participate in care planning meetings and support safe, community-based living
Required Qualifications
Active NP license in Michigan
National NP certification
Experience in geriatrics, primary care, or chronic disease management preferred
Strong clinical judgment, communication, and patient-centered care skills
Work Schedule
Full-time schedule with flexibility to support participant needs
Compensation & Benefits
Salary: $140,000–$160,000 annually
