Description
Position Overview NorTek Medical Staffing is seeking a skilled and compassionate Home Health RN Flex to provide professional nursing care directly in patients’ homes. This role requires adherence to physician orders, compliance with Medicare Conditions of Participation, Pennsylvania licensure laws, and agency policies.
Reporting to the Home Health Clinical Manager, the RN Flex conducts patient admissions (Start of Care – SOC, and Resumption of Care – ROC) and revisits, ensuring safe, high-quality, patient-centered home health services. Strong collaboration with the interdisciplinary team is essential to meet patient care needs effectively.
Key Responsibilities
Patient Assessment & Care Planning
Conduct thorough, accurate, and compassionate patient assessments
Identify active and potential patient needs impacting the plan of care
Collaborate with physicians to develop and implement medical and nursing care plans
Participate in case conferences and ongoing care planning
Care Implementation & Evaluation
Provide nursing interventions, including preventive and therapeutic procedures
Continuously assess patients, interpret findings, initiate interventions, and evaluate outcomes
Perform specialized procedures when trained and clinically qualified
Educate patients and families to support continuity of care and positive outcomes
Utilize agency services and community resources appropriately
Maintain timely, accurate, and complete clinical documentation
Meet productivity standards based on patient acuity and clinical expectations
Education & Mentorship
Serve as a mentor and resource for therapy staff, Home Health Aides, Medical Social Workers, and students
Participate in orientation, field training, and in-service education
Educate patients and caregivers on disease management, medications, diet, and self-care
Supervise Home Health Aides every 14 days in accordance with Medicare guidelines
Promote professional growth and continuous improvement
Leadership & Professional Practice
Demonstrate ethical, professional, and accountable behavior
Apply critical thinking in care planning and case management
Facilitate interdisciplinary care coordination
Function independently while seeking guidance when appropriate
Operations & Compliance
Coordinate visits efficiently and cost-effectively
Maintain necessary supplies and equipment
Ensure accurate documentation per agency standards
Participate in evening and weekend on-call rotation as required
Comply with all local, state, federal, and Medicare regulations
Additional Responsibilities
Provide nursing visits for the Hospice program as needed
Participate in staff meetings, in-services, and committee activities
Perform other duties as assigned
Qualifications
Graduate of an accredited Registered Nursing program; BSN preferred
Minimum of one year of general nursing experience; home health, medical-surgical, or critical care experience preferred
Current Registered Nurse license in Pennsylvania
CPR certification required
Valid driver’s license, reliable transportation, and auto insurance
Satisfactory criminal background check
Physical Requirements
Ability to communicate effectively both verbally and in writing
Manual dexterity for documentation and phone use
Ability to work in patients’ homes and varied environments
Must travel throughout Adams County and Southern York County
