🤝 Family-Centered Care: 1:1 care focusing on comfort, symptom management, and emotional support for families.
🩺 Role Highlights:
Perform comprehensive assessments and manage symptoms for hospice and palliative patients.
On-Call Responsibility: 1–3 shifts per week; must be able to respond to patient needs within 45 minutes.
Weekend Rotation: Includes 3 weekends of call over the 13-week contract.
EMR: Primarily uses Hospicesoft or Careficient for point-of-care documentation.
📋 Requirements:
Experience: Minimum 3 years of RN experience (Hospice or Home Health preferred).
Active License: Utah RN License (Utah is a Compact/NLC state).
Transportation: Valid driver’s license and reliable vehicle (essential for rural home visits).
Schedule: Must be available for holidays and on-call rotations.
