Shift Time: DAY SHIFT 9:00 AM-5:00 PM

Job Summary: The Adult Care Registered Nurse provides skilled nursing services to adult patients in a home-based care setting. This role focuses on assessment, care coordination, and delivery of patient-centered nursing care to support health and functional independence.

Responsibilities
• Perform comprehensive patient assessments and develop individualized care plans.
• Deliver skilled nursing care in the home setting, including medication management and monitoring.
• Monitor patient status, identify changes in condition, and communicate findings to providers.
• Educate patients and families on care plans, medications, and health management.
• Coordinate care with physicians and interdisciplinary team members.
• Document nursing care accurately and timely in the medical record.

Requirements
• Minimum 2 years of prior acute care experience in Med/Surg, Maternal & Child Care, Critical Care, or Pediatrics – Required
• Current Adult Care home health experience – Required

Education: Bachelor of Science in Nursing (BSN) – Required

Certifications
• AHA Basic Life Support (BLS) – Required
• AHA Advanced Cardiac Life Support (ACLS) – Required (Queens only)

License
• Active New York State Registered Nurse (RN) License – Required
• Primary Source Verification – Required
