📋 **Position Details:**
**Specialty:** Physical Therapist (Home Health)
**Primary Location:** Sunnyvale, CA
**Territory:** Includes Sunnyvale, Los Gatos, Campbell, San Jose, Milpitas, and neighboring cities.
**Contract:** 13 Weeks
**Schedule:** 40 hours per week | Daytime hours

🩺 **Key Clinical Responsibilities:**
**Comprehensive Evaluation:** Perform initial admission and evaluation visits, documenting observations, clinical findings, and discharge plans.
**OASIS Management:** Maintain clinical accuracy by notifying Team Managers of OASIS scores within established timelines.
**Clinical Supervision:** Provide leadership and direction to Physical Therapy Assistants (PTAs) to ensure seamless patient care.
**Documentation:** Expertly document skilled interventions, treatments, and goals in alignment with Pathways’ mission.

**🩺 Highlighted Requirements:**

- **Experience:** Minimum **3 years of mandatory Physical Therapy experience** is required (Home Health experience highly preferred).
- **Licensure:** Active **California PT License** must be in hand at the time of submittal.
- **Technical Skills:** Proficiency in **OASIS** documentation and home health EMR systems.
- **Transport:** Must have a **reliable vehicle** and valid driver’s license for daily travel across the territory.
- **Certs:** Current AHA **BLS** is mandatory.
- **Onboarding:** All credentials must be finalized and uploaded 5 business days prior to your start date.
