✨ Assignment Overview
📅 Duration: 13 Weeks | Start Date: ASAP

☀️ Schedule: 8:00 AM – 4:00 PM (Monday – Friday)

🗓️ Requirements: Some weekend rotations and on-call shifts are required.

🏥 Setting: Home Health & Hospice (VNAHSR)

💻 EMR: McKesson / Homecare Homebase (Knowledge of "locking down" visits and McKesson sync processes is essential).

🩺 Role Highlights
OASIS Expert: Conduct comprehensive assessments (Start of Care, Recertification, Discharge) using OASIS-E standards with high accuracy and timeliness.

Case Management: Manage a territorial caseload, coordinating with LPNs, Physical Therapists, and Social Workers to ensure a holistic care plan.

High-Tech Clinical Skills: Utilize advanced skills including IV maintenance, Wound VAC therapy, and complex dressing changes in the home environment.

Patient Education: Act as the primary educator for families on medication side effects, symptom management, and reportable conditions.

📋 Mandatory Requirements
Experience: Minimum 2 years of Home Health experience with a strong background in OASIS documentation.

Licensure: Active Vermont (VT) or Compact/Multi-State RN License.

Certification: Current AHA BLS.

Tech Proficiency: Experience with McKesson or similar home health EMR systems.

Compliance: Vermont requires a fingerprint-based background check, which can take 4–6 weeks—ASAP starts often utilize a 90-day provisional license while this clears.
