Looking for an impactful role in the scenic Green Mountains? Join a premier Visiting Nurse Association (VNAHSR) team in Bennington. This 13-week assignment is perfect for a highly autonomous RN who excels in case management, patient advocacy, and clinical independence.

📅 Duration: 13 Weeks | ASAP Start
☀️ Schedule: Day Shift | 37 Hours Guaranteed
🚗 Setting: Field-based (Home Health & Hospice visits)
📍 Location: Bennington, VT (Near the NY/MA border)
🩺 Role Highlights
Full-Scale Case Management: Manage a dedicated caseload, coordinate visit times, and oversee interdisciplinary needs (PT, Social Work, etc.).
Advanced Clinical Skills: Maintain expertise in high-tech interventions including IV maintenance, Wound VACs, and complex wound care.
OASIS & McKesson Mastery: Ensure timely and accurate OASIS documentation and McKesson system compliance (locking visits, follow-up notes).
IDT Leadership: Lead Interdisciplinary Team (IDT) meetings and collaborate with LPNs and HCAs to ensure the Plan of Care is executed seamlessly.
Hospice Integration: Provide symptom management and pain control education for hospice patients and their caregivers.
📋 Key Requirements
Licensure: Active Vermont (VT) RN License.
Experience: Strong background in Home Health/Hospice Case Management.
Documentation Skills: Proficiency in OASIS and McKesson software is essential.
Regulatory Knowledge: Deep understanding of Medicare Notice of Non-Coverage, HHABN, and State/Federal home care regulations.
Interpersonal Skills: Ability to perform patient/caregiver education on medication side effects, discharge instructions, and reportable conditions.
