Restore function and maximize independence for patients in one of the most iconic territories in the country. This role with Pathways Home Health & Hospice offers a premium pay package for a Physical Therapist who excels in the autonomous, fast-paced world of Bay Area home care.
✨ Financial Breakdown (Weekly)
Taxable Pay: $1,200.00
Non-Taxable Stipends: $1,988.00
Total Gross Weekly: $3,188.00
✨ Assignment Overview
📅 Duration: 13 Weeks | Start Date: 3/23/2026
☀️ Schedule: Days (8:00 AM – 5:30 PM) | Flexible based on patient availability.
🚗 Territory: San Francisco, Daly City, Brisbane, Colma, Pacifica, Millbrae, Hillsborough, and Burlingame.
📍 Headquarters: Sunnyvale, CA
🩺 Role Highlights
Clinical Restoration: Evaluate and treat patients with medically prescribed programs to restore function and minimize disability.
OASIS Expert: Complete initial admission and evaluation visits, documenting and notifying Team Managers of OASIS scores within strict timelines.
Collaborative Leadership: Supervise Physical Therapy Assistants (PTAs) and align treatment goals with the broader mission of Pathways Home Health & Hospice.
Detailed Documentation: Maintain precise records of skilled interventions, observations, clinical findings, and discharge plans.
📋 Mandatory Requirements
Experience: Minimum one year of recent Home Health experience is required.
Licensure: Active California Physical Therapy License.
Driver’s License: Valid driver's license and reliable transportation for a heavy-driving territory.
Soft Skills: Ability to work independently and manage a complex patient schedule across multiple cities.

Housing Tip: Because your territory is primarily San Francisco and the northern Peninsula, searching for housing in Daly City, South San Francisco, or Pacifica may offer better parking and slightly more competitive rates than SF proper.
Commute: Be prepared for significant city driving and varied parking conditions throughout San Francisco and coastal neighborhoods.
