🏠 RN Case Manager | Home Health & Hospice📍 Location: Lebanon, NH (Serving VT & NH Border Towns)🏢 Facility: Visiting Nurse and Hospice for VT & NH (VNH)☀️ Shift: 08:00 – 16:30 (Monday–Friday)🔄 Schedule: Full-Time with Rotating Weekends⏳ Duration: 16-Week Contract (Extended Stability)💰 Compensation & Contract OverviewThis premium New England package is designed for an experienced clinician ready to manage a diverse caseload across the beautiful Upper Valley.💵 Total Weekly Gross: $3,512.00📑 Taxable Pay: $2,000.00🏠 Non-Taxable Stipend: $1,512.00⏰ Hours: 40 hours per week🚀 Start Date: ASAP📋 Position SummaryJoin a dedicated team delivering superior nursing, hospice, and rehabilitative care to over 70 towns across Vermont and New Hampshire. As an RN Case Manager, you aren't just a visitor; you are an integral part of the patient's care team, providing the clinical expertise and emotional support that allows patients to remain in their homes with dignity.🌟 Key Responsibilities🗺️ Case Management: Screen, evaluate, and treat patients while developing comprehensive care plans in collaboration with physicians and families.🤝 Interdisciplinary Coordination: Act as the hub for the patient's care, communicating with therapists, social workers, and primary caregivers.🩺 Clinical Assessment: Provide ongoing evaluation of functional and emotional status, identifying progress or the need for plan revision.👩🏫 Education: Teach patients and families self-care techniques, medication management, and dietary instructions as ordered.📍 Territory Coverage: Provide services in areas including Lebanon (NH), White River Junction (VT), Springfield (VT), and surrounding communities.🎓 Minimum Qualifications🎓 Education: Graduate of an accredited school of nursing.💼 Experience: Minimum 2 years of dedicated experience in Home Health and Hospice.🆔 Licensure: Must hold active RN licenses in BOTH Vermont and New Hampshire or a valid unencumbered Compact License.🚗 Logistics: Valid driver’s license, reliable transportation, and auto insurance (Travel between towns is required).🌲 Why the Upper Valley (NH/VT)?The Lebanon/White River Junction area is the cultural and economic heart of the region.⛰️ Outdoor Paradise: Located at the intersection of I-89 and I-91, you have immediate access to the Green Mountains (VT) and the White Mountains (NH) for world-class skiing, hiking, and leaf-peeping.🎓 Ivy League Influence: Home to Dartmouth College, the area offers high-end dining, boutique shopping, and a sophisticated arts scene.⚖️ Work/Life Balance: This role is designed for nurses who value independence and the ability to breathe in the fresh mountain air between patient visits.
