• Evaluate and treat patients in a Skilled Nursing Facility
• Develop individualized treatment plans
• Collaborate with interdisciplinary care teams
• Complete documentation using Net Health
✅ Requirements:
• Active California Physical Therapist License
• SNF experience preferred (not required)
• 2+ year of experience required
• Strong documentation & time management skills
• Team-oriented and adaptable mindset
📍 Facility:
Salinas Valley Post Acute
(May float to a nearby sister facility within walking distance)
💡 Additional Details:
• No on-call required
• No holiday requirement
• Overtime after 40 hours
