📍 Location: Grand Junction, CO
📅 Start Date: 05/04/2026 (Quick Start!)
⏳ Duration: 13 Weeks
🕒 Schedule: Monday–Friday | 8:00 AM – 5:00 PM

💡 About the Role:
Join the PACE (Program of All-Inclusive Care for the Elderly) team and deliver compassionate, coordinated care that focuses on the whole person. You’ll play a key role in helping seniors maintain independence and quality of life.

🩺 What You’ll Do:
✔️ Assess patient needs & develop care plans
✔️ Provide direct in-home nursing care
✔️ Coordinate with interdisciplinary teams
✔️ Educate patients & families
✔️ Manage scheduling, documentation & follow-ups
✔️ Support preventive & rehabilitative care

✅ Requirements:
• Active RN License
• 1–2+ years recent acute care experience
• Home Health or LTC experience preferred
• BLS/CPR Certification
• Valid driver’s license + reliable transportation 🚗
• Strong communication & time management skills
