🕒 Schedule: Sunday–Thursday | 8:00 AM – 4:00 PM
📞 On-Call: Some required

💡 About the Role:
As a Home Health RN, you’ll serve as a case manager, delivering patient-centered care right where it matters most—at home. You’ll play a key role in assessment, planning, coordination, and education for patients and families.

🩺 What You’ll Do:
✔️ Perform comprehensive patient assessments
✔️ Develop & manage individualized care plans
✔️ Complete accurate OASIS documentation
✔️ Provide wound care, IV therapy & high-tech nursing skills
✔️ Educate patients & caregivers
✔️ Coordinate care with interdisciplinary teams
✔️ Ensure compliance with state & federal guidelines

✅ Requirements:
• Active RN License (or willing to obtain upon offer)
• Minimum 2+ years experience
• OASIS experience REQUIRED
• Strong case management & documentation skills
• Home Health experience preferred
