Job Title: Registered Nurse (RN) – Home Health / Hospice
Job Type: Contract
Location: Gunnison, Colorado
Start Date: ASAP
Duration: 13 Weeks
Shift: Day Shift with On-Call
Schedule: 36 hours per week
Job Summary:
We are seeking an experienced Home Health/Hospice Registered Nurse (RN) to provide comprehensive patient care and case management. This role involves delivering skilled nursing services, coordinating care, and supporting patients and families in home health, hospice, and palliative care settings.
Key Responsibilities:
Perform initial patient assessments and ongoing re-evaluations
Develop, implement, and update individualized care plans
Provide skilled nursing care and interventions
Monitor patient conditions and communicate changes to physicians
Coordinate care with interdisciplinary teams
Educate and counsel patients and families on care needs
Maintain accurate clinical documentation and progress notes
Participate in in-service training and professional development
Required Qualifications:
Active RN License (state or compact)
Minimum 3 years of nursing experience
Experience in home health, hospice, or palliative care preferred
Required Certifications:
BLS (Basic Life Support)
