🌟 Occupational Therapist (OT) – Home Health & Hospice | Contract Assignment 🌟

📍 Location: St Johnsbury, VT 05819
📅 13-Week Contract | Start ASAP
🕒 Schedule: Monday – Friday | Up to 40 Hours/Week

✨ Job Summary:
Seeking a compassionate and independent Occupational Therapist to provide in-home therapy services focused on maximizing patient independence, mobility, and safety for home care and hospice patients. This role requires strong clinical judgment and the ability to work collaboratively with an interdisciplinary team.

🔹 Must-Have Skills:
✔ Active Vermont OT License
✔ Valid Driver’s License
✔ Reliable Personal Vehicle
✔ Strong communication and patient care skills
✔ Ability to work independently in home settings

🔹 Experience Required:
✔ Minimum Less than 1 year of Occupational Therapy experience required
✔ Home Health experience preferred
✔ Hospice experience preferred

🔹 Key Responsibilities:
• Evaluate patients in their home environment
• Develop individualized occupational therapy treatment plans
• Improve patient independence with daily living activities
• Recommend adaptive equipment and home safety modifications
• Maintain accurate documentation in EMR
• Coordinate care with nurses, physicians, and interdisciplinary team
