✨ Job Summary:
Seeking a dedicated Physical Therapist (PT) to provide rehabilitation services in a Skilled Nursing Facility setting. The PT will evaluate residents, develop individualized treatment plans, and provide therapy focused on improving mobility, strength, balance, and overall functional independence.

🔹 Must-Have Skills:
✔ Active California Physical Therapist License
✔ Strong patient assessment and treatment planning skills
✔ Ability to work in Skilled Nursing / Rehab settings
✔ Excellent communication and teamwork skills
✔ Accurate clinical documentation skills

🔹 Experience Required:
✔ Minimum Less than 1 year of PT experience required
✔ Skilled Nursing Facility experience preferred
✔ New graduates encouraged to apply based on experience level

🔹 Key Responsibilities:
• Evaluate residents and create individualized treatment plans
• Provide therapeutic exercise, mobility, gait, and balance training
• Monitor progress and modify treatment plans as needed
• Educate residents and caregivers on exercises and safety strategies
• Maintain timely and accurate documentation
• Collaborate with rehab team and interdisciplinary staff
• Promote safe functional independence and quality outcomes
