⏰ Guaranteed Hours: 38 Hours/Week

✨ Job Summary:
Seeking an experienced Physical Therapist (PT) to provide rehabilitation services in a Skilled Nursing Facility setting. The PT will evaluate residents, create individualized treatment plans, and deliver therapy focused on improving mobility, strength, balance, and functional independence.

🔹 Must-Have Skills:
✔ Active California Physical Therapist License
✔ Strong patient assessment and treatment planning skills
✔ Skilled Nursing Facility rehab experience
✔ Excellent communication and teamwork skills
✔ Accurate documentation skills

🔹 Experience Required:
✔ Minimum 3 year of PT experience required
✔ At least 1 year SNF experience preferred
✔ NetHealth / Rehab Optima knowledge preferred

🔹 Key Responsibilities:
• Evaluate residents and develop individualized treatment plans
• Provide therapeutic exercise, gait, balance, and mobility training
• Monitor progress and modify treatment plans as needed
• Educate residents and caregivers on safety and home exercise programs
• Maintain timely and accurate documentation
• Collaborate with rehab and interdisciplinary care teams
• Promote resident independence and quality outcomes

CloudinfraIT Inc Job ID #295965. Pay package is based on 12 hour shifts and 38 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
