✨ Job Summary:
Seeking a motivated Occupational Therapist (OT) to provide outpatient rehabilitation services in a Critical Access Hospital setting. The OT will evaluate patients, develop treatment plans, and deliver therapy focused on improving functional independence and daily living skills.

🔹 Must-Have Skills:
✔ Active Illinois Occupational Therapist License
✔ Strong patient assessment and treatment planning skills
✔ Experience in outpatient rehabilitation settings
✔ Knowledge of ADL training and therapeutic interventions
✔ Excellent communication and teamwork skills

🔹 Experience Required:
✔ Minimum 3 year of OT experience required
✔ Outpatient rehab experience preferred

🔹 Key Responsibilities:
• Evaluate patients and develop individualized treatment plans
• Provide therapy focused on functional independence and ADLs
• Monitor patient progress and modify treatment plans as needed
• Educate patients and caregivers on therapy techniques
• Maintain accurate and timely documentation in EMR
• Collaborate with interdisciplinary care team
• Ensure compliance with facility policies and procedures
