⏰ Guaranteed Hours: 40 Hours/Week

✨ Job Summary:
Seeking an experienced Physical Therapist (PT) to provide rehabilitation services in a Skilled Nursing Facility (SNF) setting. The PT will work closely with residents and interdisciplinary teams to improve mobility, functional independence, and quality of life.

🔹 Must-Have Skills:
✔ Active Massachusetts Physical Therapist License
✔ Strong SNF / Long-Term Care experience (required)
✔ Excellent patient assessment and treatment planning skills
✔ Knowledge of Medicare and rehabilitation documentation standards
✔ Strong communication and teamwork abilities

🔹 Experience Required:
✔ Minimum 3 years PT experience required
✔ Skilled Nursing / LTC experience required

🔹 Key Responsibilities:
• Evaluate residents and develop individualized treatment plans
• Deliver therapy to improve mobility, strength, and ADLs
• Monitor patient progress and adjust treatment plans
• Collaborate with physicians and interdisciplinary teams
• Educate residents and caregivers on therapy goals and exercises
• Maintain accurate EMR documentation and compliance with reimbursement guidelines
• Supervise PTA staff and support team-based care
