⏰ Guaranteed Hours: 36 Hours/Week

🩺 Job Summary:

Seeking a motivated Physical Therapist (PT) to provide rehabilitation services in a Skilled Nursing Facility (SNF) / Long-Term Care setting. The PT will work with patients to improve mobility, strength, and functional independence while collaborating with interdisciplinary teams.

✅ Must-Have Skills:

✔ Active Wyoming Physical Therapist License
✔ Strong clinical assessment and treatment planning skills
✔ Experience in SNF / Long-Term Care settings
✔ Ability to work independently and within a team

📌 Experience Required:

✔ Minimum 3 year Physical Therapist experience
✔ SNF or rehab experience preferred

🔑 Key Responsibilities:

• Evaluate patients and develop individualized therapy plans
• Provide treatments to improve mobility and functional outcomes
• Monitor patient progress and modify plans as needed
• Collaborate with physicians and interdisciplinary teams
• Maintain accurate and timely documentation
• Educate patients and caregivers on rehabilitation goals
• Ensure compliance with facility policies and standards
