⏰ Guaranteed Hours: 30 Hours/Week

🏥 Job Summary:

We are seeking a dedicated Physical Therapist (PT) to work in a Skilled Nursing Facility (SNF) setting. The PT will be responsible for evaluating patients, developing treatment plans, and helping patients improve mobility, strength, and overall functional independence.

✅ Must-Have Skills:

✔ Active California Physical Therapist License
✔ Strong clinical assessment and treatment planning skills
✔ Ability to work in a SNF / long-term care environment
✔ Excellent communication and teamwork skills

📌 Experience Required:

✔ Minimum 3 years of experience required
✔ SNF experience preferred but not mandatory

🔑 Key Responsibilities:

• Evaluate patients and create individualized treatment plans
• Provide therapy to improve mobility and functional outcomes
• Monitor patient progress and adjust care plans as needed
• Collaborate with interdisciplinary teams
• Maintain accurate and timely documentation
• Educate patients and caregivers on therapy goals
