🌟 Travel Occupational Therapist (OT) – Home Health | 13 Weeks Contract 🌟
📍 Location: Sunnyvale, CA 94085
⏳ Duration: 13 Weeks
🕒 Shift: Day Shift (Monday–Friday, Flexible Hours between 8:30 AM – 5:00 PM)

🏥 Job Summary:
We are seeking a dedicated Occupational Therapist (OT) for a Home Health setting to help patients restore function, improve independence, and enhance their quality of life. This role focuses on evaluating and treating patients using individualized therapy programs in their home environment.

✔ Redwood City
✔ Nearby surrounding cities as needed

✅ Must-Have Skills:
✔ Active California OT License
✔ Strong Home Health or Rehabilitation experience
✔ Ability to work independently and manage caseload
✔ Strong patient assessment and care planning skills
✔ Excellent communication and documentation skills

📌 Experience Required:
✔ Minimum 3 year of Occupational Therapy experience
✔ Home Health experience preferred

🔑 Key Responsibilities:
• Evaluate and treat patients using physician-prescribed therapy plans
• Develop individualized care plans to improve daily living activities
• Help patients regain independence and functional mobility
• Educate patients and families on exercises, safety, and care techniques
• Maintain accurate documentation in EMR (Homecare Homebase preferred)
• Collaborate with interdisciplinary healthcare teams
• Monitor patient progress and adjust treatment plans accordingly

📜 Certifications Required:
✔ Relevant OT Licensure
✔ BLS (if required by facility)
