💥 Responsibilities Include:
• Developing individualized hospice care plans
• Coordinating care with physicians & interdisciplinary teams
• Ongoing patient assessments & symptom management
• Educating patients & caregivers on end-of-life care
• Providing compassionate emotional support to patients and families
• Accurate clinical documentation & case management

✅ Requirements:
• Active Vermont & New Hampshire RN License OR Compact License
• Graduate of an Accredited Nursing Program
• Minimum 2 Years RN Experience
• BLS Required
• Hospice, Oncology, or Med/Surg Experience Preferred
