Description

**Travel Hospice RN Case Manager | Montrose, CO**
**13 Weeks | Days | Mon-Fri | 8:00 AM – 5:00 PM**

We are seeking a compassionate Hospice RN Case Manager to provide individualized end-of-life care to patients in homes, assisted living facilities, and nursing facilities.

**Requirements:**
* Active RN CO License
* Minimum 3 years of Hospice RN experience
* Strong assessment, symptom management, and case management skills
* Experience managing an assigned patient caseload and interdisciplinary team collaboration

**Assignment Details:**
* 13-week contract
* Monday-Friday, 8:00 AM – 5:00 PM
* Rotating weekend and on-call coverage required
* Holiday coverage required
* No guaranteed hours

**Responsibilities:**
* Provide compassionate hospice and end-of-life care
* Manage patient caseload and coordinate care plans
* Educate patients and caregivers on symptom management and goals of care
* Collaborate with physicians and interdisciplinary teams

CloudinfraIT Inc Job ID #297237. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
