**Travel RN – Home Health & Hospice | Vermont**
**12-Week Contract | Day Shifts | 36 Guaranteed Hours**

Seeking a compassionate **Home Health & Hospice Registered Nurse** for a travel assignment with **Northern Counties Health Care** in Vermont. This role provides comprehensive nursing care to patients of all ages in their homes while collaborating with an interdisciplinary team to deliver high-quality, patient-centered home health and hospice services.

**Requirements:**
* Active Vermont RN License
* Minimum 3 year of home health /hospice experience
* Home Health and/or Hospice experience
* Experience with Homecare Homebase (HCHB) EMR
* Strong assessment, care planning, and communication skills
* Ability to work independently in a home care setting

**Assignment Details:**
* 12-week contract
* Day shifts, Monday–Friday
* Possible weekend shifts
* 36 guaranteed hours per week
* No on-call
* No holiday coverage required
* W-2 employment only (1099 not accepted)
* Mileage reimbursement at the current facility rate ($0.50/mile)

**Responsibilities:**
* Complete patient admissions, assessments, and ongoing evaluations
* Develop and implement individualized home health and hospice care plans
* Provide skilled nursing care and symptom management in patients' homes
* Educate patients and families regarding treatments, medications, and disease management
* Coordinate care with physicians and the interdisciplinary healthcare team
* Document patient care accurately using Homecare Homebase (HCHB)
* Monitor patient progress and update care plans as needed
* Deliver compassionate, high-quality care while promoting patient comfort and independence
