💙 Quick Start Opportunity for Experienced Home Health/Hospice RNs! Join a dedicated team providing compassionate, patient-centered care in the comfort of patients' homes.

📍 Location: Missoula, Montana
📅 Start Date: ASAP
⏳ Contract Length: 13 Weeks
🕒 Schedule: Typically Monday–Friday, 8:00 AM–5:00 PM (varies) with on-call rotation
💼 Guaranteed Hours: 40 hours/week
🖥️ EMR: Homecare Homebase (HCHB)

Requirements:
✅ Active Montana RN License
✅ Minimum 3 years of recent Home Health, Hospice, or Acute Care experience
✅ Valid driver's license, reliable transportation, and auto insurance
✅ Home Health or Hospice experience preferred
Responsibilities:
Provide skilled nursing care and case management in patients' homes
Complete patient assessments, admissions, and individualized care plans
Coordinate care with interdisciplinary teams to ensure continuity
Educate patients and families on treatment plans and symptom management
Maintain accurate documentation in Homecare Homebase (HCHB)
Participate in on-call rotation and quality improvement initiatives
