Travel Physical Therapist – Home Health | New Mexico
Albuquerque, NM Area
13-Week Contract | Monday–Friday | 30 Guaranteed Hours/Week

We’re hiring an experienced Physical Therapist (PT) for a home health travel assignment in New Mexico!

🔹 Requirements:
• Minimum 3 years of Physical Therapy experience required
• Home health experience preferred but not required
• Graduate of an accredited Physical Therapy program
• Active New Mexico Physical Therapist License
• Current CPR certification
• Strong clinical judgment and ability to work independently
• Experience with individualized treatment plans and skilled interventions
• Knowledge of OASIS assessments and documentation
• Familiarity with ICD-10 coding and compliant clinical documentation
• Strong communication and interpersonal skills
• Basic computer/EMR proficiency
• Valid driver’s license, reliable transportation, and auto insurance required
• Ability to travel for home visits within the assigned service area
• Commitment to Medicare/Medicaid, HIPAA, safety, and infection-control requirements

🔹 Assignment Details:
• 13-week contract
• 30 guaranteed hours/week
• Monday–Friday schedule
• Quick Start – 8/31/2026
• Home health visits within approximately a 45-mile radius of the Albuquerque area
• Service area may include Albuquerque, Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen
• Some SNF locations may also be included
• No on-call requirement
• No holiday coverage required
• 1 position available
• W-2 employment only
• Mileage reimbursement is not included in the posted rate

🔹 Key Responsibilities:
• Conduct initial evaluations and ongoing patient assessments
• Develop and update individualized plans of care
• Provide skilled physical therapy and maintenance interventions
• Complete progress notes, reassessments, recertifications, transfers, and discharges
• Complete required OASIS documentation within agency timelines
• Apply appropriate ICD-10 coding
• Educate patients and caregivers on home exercise programs, mobility, and safety
• Promote patient independence and functional improvement
• Supervise PTAs and Home Health Aides as permitted by state regulations
• Coordinate care with physicians and interdisciplinary team members
• Participate in QAPI activities, case reviews, and staff education
• Maintain accurate and timely clinical documentation
• Follow HIPAA, infection-control, safety, and risk-prevention standards
• Provide professional and compassionate care in patients’ homes
