We’re hiring an experienced **Hospice Registered Nurse (RN Case Manager)** for a travel assignment in California!

🔹 **Requirements:**
• **Minimum 3 years of Hospice nursing experience required**
• Active **California RN License**
• Current **BLS**
• Strong hospice and end-of-life care experience
• Excellent assessment, communication, and case-management skills
• Ability to work independently in a home-based care setting
• Strong patient and family education skills

🔹 **Assignment Details:**
• **13-week contract**
• **Day shift: 8:30 AM–5:00 PM**
• Flexible schedule based on patient availability
• **One weekend per month**
• **40 hours/week**
• Holiday coverage required
• **Homecare Homebase (HCHB) EMR**
• **Mileage reimbursement: $0.76/mile**
• W-2 employment only
• No on-call

🔹 **Territory:**
• San Francisco
• Brisbane
• Daly City
• Colma
• South San Francisco
• Pacifica
• Neighboring areas as needed

🔹 **Key Responsibilities:**
• Admit patients to hospice services
• Perform comprehensive physical, psychosocial, environmental, safety, and developmental assessments
• Develop individualized plans of care
• Provide skilled nursing and end-of-life care
• Monitor patient symptoms and changes in condition
• Coordinate care with physicians and multidisciplinary teams
• Educate patients and families regarding hospice services and symptom management
• Conduct follow-up and routine hospice visits
• Document assessments, interventions, and patient responses
• Promote patient comfort, dignity, and quality of life
• Participate in quality assessment and performance improvement initiatives
• Communicate effectively with caregivers and the healthcare team
• Ensure care is delivered according to hospice standards and the individualized plan of care
