📍 Grand Junction, Colorado
⏳ Contract: 13 weeks
🕐 Shift: Flexible Day Schedule
📆 Schedule: Monday–Friday + On-Call Rotation
⏱️ Guaranteed Hours: 40 hours/week

Position Overview

Seeking a dependable and compassionate Home Health RN to provide skilled, patient-centered nursing care to patients in their homes. This position offers independent visits, flexible daily workflows, and collaboration with an interdisciplinary care team.

Responsibilities
Perform comprehensive patient assessments
Develop and update individualized plans of care
Provide skilled nursing care and appropriate interventions
Monitor patient progress and report changes in condition
Educate patients and families on medications, treatments, and care plans
Coordinate care with LPNs, therapists, caregivers, and providers
Complete timely and accurate documentation in the EMR
Manage an assigned caseload independently
Participate in the on-call rotation
Maintain effective communication with patients and the healthcare team
Requirements

✅ Active Colorado RN License or active NLC Compact License
✅ Valid driver's license
✅ Reliable transportation and auto insurance
✅ Strong assessment and critical-thinking skills
✅ Ability to work independently and manage a caseload
✅ Strong communication and organizational skills
✅ Comfortable using EMR systems
✅ Homecare Homebase (HCHB) experience preferred
✅ Home Health experience preferred, but not required
