Position Overview

Seeking a compassionate and experienced Occupational Therapist (OT) to provide meaningful, patient-centered therapy in a home health setting. The OT will work independently while collaborating closely with an interdisciplinary care team.

Responsibilities
Assess patients' functional abilities and needs
Perform evaluations and re-evaluations
Provide individualized occupational therapy treatments
Select appropriate therapeutic modalities and interventions
Develop treatment goals and objectives
Monitor patient progress and adjust treatment plans as needed
Educate patients and family members regarding treatments and procedures
Communicate effectively with the interdisciplinary healthcare team
Maintain accurate documentation of evaluations, treatments, goals, and progress
Coordinate patient care with other healthcare professionals
Promote patient safety, independence, and functional improvement
Requirements

✅ Active Vermont and/or New Hampshire OT License or unencumbered compact license
✅ Bachelor's degree in Occupational Therapy or postgraduate OT certification
✅ Successful completion of the National Registration Exam
✅ Minimum 3 year of experience
✅ Home Health experience preferred
✅ Strong communication and interpersonal skills
✅ Ability to work independently in patients' homes
✅ Patient-centered approach and strong clinical judgment
