Travel Physical Therapist Assistant (PTA) – Home Health | Vermont/New Hampshire
White River Junction, VT
13-Week Contract | Day Shift | 36 Hours/Week

We’re hiring an experienced Physical Therapist Assistant (PTA) for a home health travel assignment!

🔹 Requirements:
• Minimum 3 years of PTA experience required
• Associate’s Degree in Physical Therapist Assistant program
• Active PTA licenses in both New Hampshire and Vermont
• Current BLS certification
• Valid driver’s license
• Valid agency-level auto insurance
• Home health experience preferred
• Strong patient-care, communication, and documentation skills
• Ability to work independently in patients’ homes
• Ability to follow plans of care established by a Registered Physical Therapist

🔹 Assignment Details:
• 13-week contract
• 36 guaranteed hours/week
• Day shift
• EPIC EMR
• No holiday coverage required
• No on-call
• Mileage reimbursement: $0.61/mile or current agency rate
• 1 position available
• W-2 employment only

🔹 Key Responsibilities:
• Provide physical therapy services under the supervision of a Registered Physical Therapist
• Treat patients according to established plans of care
• Assist with exercises, muscle re-education, gait and functional training
• Provide transfer training and assistance with activities of daily living
• Support prosthetic training as appropriate
• Monitor and report patient responses and changes in condition
• Teach patients and caregivers self-care techniques and therapeutic activities
• Assist patients with maintaining and improving functional performance
• Coordinate equipment needs for home-based care
• Complete accurate clinical notes and required documentation
• Communicate patient progress and concerns to the supervising PT
• Follow agency policies, state regulations, safety, and infection-control standards
