We’re hiring an experienced Physical Therapist (PT) for a skilled nursing facility travel assignment in California!

🔹 Requirements:
• Minimum 3 years of Physical Therapy experience required
• Active California Physical Therapist License
• Graduate of an accredited Physical Therapy program
• Ability to provide skilled PT services in a Skilled Nursing Facility (SNF) setting
• Strong clinical assessment and treatment-planning skills
• Experience developing and implementing individualized plans of care
• Ability to evaluate patient mobility, strength, balance, and functional limitations
• Skilled in therapeutic exercises, gait training, transfer training, and functional mobility
• Strong patient-care, communication, and documentation skills
• Ability to work independently and effectively as part of an interdisciplinary team
• Must maintain accurate and timely clinical documentation
• Commitment to patient safety, quality care, and professional standards

🔹 Assignment Details:
• 13-week contract
• 40 hours/week
• Monday–Friday
• Day shift
• Skilled Nursing Facility setting
• 1 position available
• No weekend requirement listed
• No holiday coverage required
• No on-call requirement
• W-2 employment only

🔹 Key Responsibilities:
• Perform comprehensive physical therapy evaluations and assessments
• Develop individualized treatment plans based on patient needs and goals
• Provide skilled therapeutic interventions to improve strength, mobility, balance, and function
• Perform gait and functional mobility training
• Provide transfer and ADL training as appropriate
• Monitor patient progress and modify treatment plans accordingly
• Educate patients and caregivers on exercises, mobility, and safety techniques
• Coordinate care with physicians, nurses, therapy staff, and other interdisciplinary team members
• Document evaluations, treatment sessions, progress, and outcomes accurately
• Ensure treatment is delivered according to the established plan of care
• Promote patient independence and safe functional mobility
• Follow infection-control, patient-safety, and facility policies
• Maintain professional and compassionate patient interactions
