🏥 **Ambulatory Clinical Pharmacy Specialist**

📍 **Location:** Dover, Delaware
💼 **Job Type:** Permanent Full-Time
🕐 **Shift:** Monday–Friday, 8:00 AM–4:30 PM or 8:30 AM–5:00 PM
🚫 **Weekends & Holidays:** None

### **Job Requirements**
* Bachelor’s Degree in Pharmacy
* Active Pharmacist License
* **BPS Certification required** in one of the following:
* Pharmacotherapy
* Ambulatory Care
* Cardiology
* Geriatrics
* Minimum **3 year of clinical pharmacy experience**, including residency experience

### **Key Responsibilities**
* Provide ambulatory clinical pharmacy services under collaborative practice agreements (CPAs)
* Conduct medication management visits, reconciliation, lab monitoring, and patient counseling
* Support chronic disease management and specialty medication care
* Coordinate prior authorizations, financial assistance, and benefit investigations
* Collaborate with providers and multidisciplinary teams
* Document clinical interventions in the electronic health record
* Participate in quality improvement, research, education, and precepting

* Permanent direct-hire position
