Seeking an Inpatient Pharmacist to provide safe, effective, and cost-efficient medication therapy while supporting patients, providers, nursing staff, and the pharmacy team.

Requirements
Pharm.D. degree and registered pharmacist licensure
Unrestricted pharmacist license in any state
Alaska pharmacist license required
BLS – American Heart Association; may be obtained within 1 year of hire
Minimum 3 years of experience
Pharmacy residency preferred
Clinical Competency required within 3 months of hire and every 3 years
High Risk Competency required within 3 months of hire and annually
Ability to travel approximately 10%

Education Note: Candidates licensed before 2003 with a Bachelor of Pharmacy degree may be considered with 3 years of hospital and/or ambulatory clinic pharmacy experience.

Responsibilities
Review prescriptions for appropriateness, safety, interactions, allergies, and contraindications
Verify and dispense medications accurately
Counsel patients regarding medications and treatment outcomes
Perform medication therapy management and chart reviews
Provide drug information and clinical consultation
Direct and educate pharmacy support personnel
Participate in quality improvement and medication safety activities
Support formulary management and pharmacy-run clinics
Participate in interdisciplinary care planning
Provide Long-Term Care pharmacy support when assigned
