📍 Location: White River Junction, VT
📅 Contract: 16 Weeks
🌞 Shift: Days | Full-Time
⏱️ Hours: 36 Hours/Week

Float location:Bradford,VT / Randolph, VT / Springfield, VT/ Lebanon, NH
We’re seeking an experienced Home Health RN Case Manager to provide patient-centered care, coordinate treatment plans, and work collaboratively with patients, families, physicians, and interdisciplinary teams.

✅ Requirements

Active Vermont & New Hampshire RN License or unencumbered Compact RN License
Minimum 3 years of Home Health RN experience
🎓 Graduate of an accredited nursing program
❤️ Current BLS/BCLS
Strong patient assessment, care coordination, and documentation skills

🩺 Key Responsibilities

Develop and manage individualized patient care plans
Perform ongoing clinical, functional, and emotional assessments
Coordinate care with physicians and interdisciplinary teams
Educate patients and caregivers on medications, diet, and self-care
Document patient care and communicate changes in condition
