### 🩺 **Position Overview**
We are seeking an experienced **Home Health Physical Therapist** to provide skilled therapy services to patients in their homes and other care settings. The PT will evaluate patients, develop individualized treatment plans, provide therapeutic interventions, and support patients in achieving safe and independent functional outcomes.

### ✅ **Requirements**
- **3 years of recent Physical Therapist experience**
- Active **Montana PT license**
- Valid driver's license
- **BLS certification**
- Home Health experience preferred
- Strong assessment, treatment planning, and documentation skills
- Ability to travel between patient homes and care settings
- Excellent communication and collaboration skills

### 📋 **Key Responsibilities**
- Review physician therapy orders for Home Health and Hospice patients
- Perform comprehensive physical therapy evaluations
- Develop individualized treatment plans with short- and long-term goals
- Provide therapeutic exercises, stretching, mobility and balance training
- Perform functional rehabilitation and pain management interventions
- Assess home safety and recommend assistive equipment or modifications
- Monitor patient progress and update treatment plans as needed
- Educate patients, caregivers, and families on home exercise programs and safety
- Complete evaluations, progress notes, recertifications, resumptions of care, and discharge documentation
- Collaborate with physicians, nurses, therapists, and other healthcare professionals
