Responsibilities:

• Evaluate patients and develop individualized treatment plans

• Collaborate with interdisciplinary healthcare teams
• Maintain accurate patient documentation and progress reports
• Educate patients and caregivers on therapy goals and home programs

Requirements:

✅ Active California Occupational Therapist License
✅ Minimum 3 Year of OT Experience
✅ Skilled Nursing Facility experience preferred
✅ Strong communication and patient care skills
