Position Overview

We are seeking an experienced Registered Nurse to serve as a Home Care Case Manager. The role involves providing comprehensive nursing care to patients in a high-volume home health setting, managing case documentation, and coordinating patient care plans. The ideal candidate has strong home health experience, case management skills, and familiarity with OASIS documentation.

Key Responsibilities

Conduct patient assessments and develop individualized care plans

Manage 5–6 patient visits per day, ensuring high-quality patient care

Perform case management duties, including coordination with interdisciplinary teams

Complete OASIS documentation and other required clinical records accurately and timely

Provide wound care when applicable

Monitor patient progress and adjust care plans as needed

Educate patients and families on health management and care instructions

Maintain compliance with regulatory, safety, and quality standards

Qualifications

Minimum 2 years of home health experience

Case management experience required

Proficiency with OASIS documentation required

Wound care experience preferred

Strong organizational and communication skills

Ability to work independently in a home care setting

Valid driver’s license and reliable transportation

Required Compliance & Credentials

Candidates must meet standard healthcare compliance requirements, including but not limited to:

RN license verification

BLS certification

Background check and drug screening

Educational and employment verification

Immunization records (MMR, Varicella, Hep B, Tdap, Influenza, COVID-19)

TB screening

Competency exam and skills checklist

Car insurance and motor vehicle report

State and federal exclusion screenings
